SMS reminders are sent 3 days prior to your appointment. Please text YES or NO or call 07 4887 5123 to confirm. If we fail to hear from you by 24 hours before your appointment, the appointment will be cancelled.

We ask that you arrive 15 minutes prior to your appointment as there are some new patient forms that need to be completed. Alternatively, the below forms can be completed and emailed to
[email protected]. This will need to be done before your appointment.

What to bring to your first appointment:

The referral from your GP or specialist.
Any relevant reports or letters , previous chest imaging , lung function tests or sleep studies relevant to your condition.
Any history of previous medications/treatments trialed.
A current medication list.
Any allergies.
Medicare Card, Pension or Concession Card if you have one, Veterans Affairs (DVA) Card if you have one Invoicing information if referred by a 3rd party such as Workcover or your employer.
